This policy describes how we handle information collected through www.camilleguzelmd.com and our interest / contact forms. It is not a HIPAA Notice of Privacy Practices. If we determine the practice is a HIPAA-covered entity, a separate Notice of Privacy Practices will also be posted.
What we collect
When you use our website forms, we may collect:

- Email address
- Phone number (if you provide it)
- Other details you choose to enter (for example, how you heard about us, or whether you are a patient, family member, or referring clinician)
Please do not include health details, diagnoses, symptoms, or medical history in forms or email. Use the form only for interest, updates, waitlist, or administrative contact.
How we use it
We use this information to:
- Respond to your inquiry
- Manage waitlist / updates requests
- Coordinate scheduling and practice logistics
- Improve how we communicate about the practice
We do not sell your information.
Where it is stored
Form responses are stored in our Google Workspace account under a Google Workspace Business Associate Agreement. Access is limited to practice team members who need it for operations.
Sharing
We share information only as needed to operate the practice (for example, with our virtual admin under appropriate agreements), when required by law, or with your direction.
Tracking
As a conservative launch rule, we keep advertising pixels, conversion tracking, session-replay tools, and third-party chat widgets off form pages.
Your choices
Email info@camilleguzelmd.com to ask us to update or delete contact information we hold from website forms, subject to any legal retention needs.
Changes
We may update this policy. The effective date above will change when we do.
